Know the questionnaire you’re working with

You need to be very familiar with the questionnaire you are using with a young person, to make sure you can use it well to support open conversation and progress with them.

On this page you’ll find an overview of each questionnaire available, and a downloadable handbook that helps you use it accurately and effectively.

Mentoring questionnaire

This questionnaire is based around nine areas of holistic health.

Who should you use it with?

This questionnaire is designed for use with young people who:

  • have been involved in the youth justice system or state care and protection
  • are between 10 and 17 years old
  • are disengaged from mainstream education.

The questionnaire supports them in improving their lives through personal growth.

What programmes use this questionnaire?

This questionnaire is used with the following programmes:

  • Rangatahi Ora (SYSCo)
  • YsUP (Group Mentoring)
  • Awa (Te Ao Māori)
  • Mentoring (Oranga Tamariki)

Youth Development questionnaire

This questionnaire focusses on mental and physical health, as well as positive connections and leadership. It covers twelve questions based on the Te Whare Tapa Whā, Circle of Courage and Strengths based models.

Who should you use it with?

This questionnaire is designed for use with young people who are:

  • engaged in listed programming for more than 12 weeks
  • between 12 and 24 years old
  • school students or young adults who are transitioning from childhood to adulthood.

This questionnaire supports them to improve their holistic wellbeing and provides a platform for youth voice and advocacy.

What programmes use this questionnaire?

This questionnaire is used with the following programmes:

  • Raise Up
  • Youth mentoring programmes
  • Peer to Peer education mentoring programme
  • Youth advisory board

Choosing a questionnaire

As we develop, ImpactStar® will include different questionnaires for working with different groups of people. When this happens, you will need to select a pre-written questionnaire (from a list in the tool) that best suits the young person and the programme.

Your manager will support you to make this decision. Make sure you check your choice with them before starting work with a young person.
